Athletic Physicals

GlenOak Sports Medicine will be offering athletic physicals at GlenOak High School on May 28, starting at 2:30 pm. The exam will be in the high school athletic training clinic, room number W115.

• Please be in line by 4:30 pm.
• The cost is $10.00, check or cash will be accepted. Checks payable to GlenOak Athletics.
• Please bring your signed physical paperwork from Final Forms with you. Instructions on how to print the document can be found here.
• Please enter through Door 17 and walk down the stairs. Turn right at the bottom of the stairs and proceed through the W-hallway until you reach W115.
• Oakwood students may ride over to GlenOak using first-wave buses departing from Oakwood on bus number 47 or 58. Students will be dropped off at door 17 at GlenOak High School.
• 6th grade Glenwood students may ride over to GlenOak on a bus at dismissal time, please email transportation@plainlocal.org to coordinate. Students will be dropped off at door 17 at GlenOak High School.
